Performance Drivers : A Practical Guide to Using the Balanced Scorecard by Nils-Göran Olve, Jan Roy, and Magnus Wetter.

It was published in 1999 (originally in Swedish), and still remains the best practical book on Balanced Scorecard going (in my view at least). Has much better examples / cases than other books, and in particular presents a fresh view on the topic compared to the Kaplan & Norton books (and those by others that are basically copies of these).
